How can I make a referral for a diagnostic evaluation?

Please have your primary care provider (PCP) or early intervention (EI) professional download and submit the referral form, along with any relevant medical and EI records. Once we have received the referral, we will be in touch to discuss intake paperwork and scheduling.

How can I prepare my child for the evaluation?

Children enjoy the evaluation experience most when they are well-rested, wearing comfortable clothing, and have eaten a good breakfast. Please bring along a water bottle, diapers/wipes, and a snack. You will stay in the room with your child throughout the evaluation.

What will happen during and after the evaluation?

The first part of the evaluation will take place over the phone or through a virtual meeting, where we will gather information about your child’s development. The second part of the evaluation will take place in our office. After greeting you and your child and getting comfortable, the evaluator will engage your child in a variety of play activities that are often fun and interesting. You may be asked to fill out some questionnaires during this time, as well as to be a support for your child. After a lunch break, we will discuss the results of the evaluation and recommendations. A full report will be sent to you and your referring provider 3-4 weeks after the evaluation.
